Medical Records / Requests. To request copies of your CMH record or to transfer care, please contact our records custodian. We will provide the required HIPAA Authorization and Transfer-of-Care forms upon request and give secure submission instructions. Patients may continue care with any provider of their choice. CMH did not transfer patient records to NCM; records are released or transferred only with patient authorization.
