Colorado Medical Health, LLC — Notice of Dissolution

Medical Records / Requests:

• To request copies of your CMH medical record or to transfer care, please contact the records custodian. Upon request, we will provide the required HIPAA Authorization and Transfer-of-Care forms along with secure submission instructions. Patients may continue care with any provider of their choice.
• CMH’s patient records are securely maintained by a designated third-party records custodian.
• These archived CMH records remain separate from any other clinical systems and are maintained in read-only form. Records are released or transferred only with patient authorization or as otherwise required by law.
• Records are maintained in accordance with HIPAA and applicable Colorado medical-records retention laws, including C.R.S. § 25-1-801.

• Northern Colorado Medical, LLC (NCM) is an independent medical practice formed after CMH ceased operations. NCM is not a successor to CMH and did not assume CMH’s liabilities, assets, or operations.
• NCM serves solely as the designated custodian of CMH’s patient records for purposes of legal and regulatory compliance. CMH records remain the records of CMH and are maintained separately in archived, read-only form. Records are released or transferred only with proper authorization or as required by law.
