Colorado Medical Health, LLC (CMH) ceased business operations at 5:00 PM on May 9, 2025, and dissolved effective May 20, 2025. CMH no longer schedules appointments or provides medical treatment.
Medical Records Requests
- Patients will be provided with the required HIPAA authorization and secure records-release instructions upon request.
- Archived CMH patient records are securely maintained by a designated records custodian in accordance with HIPAA and applicable Colorado medical-records retention requirements, including C.R.S. § 25-1-801.
- CMH records are maintained in archived, read-only form and are released only with proper authorization or as otherwise required by law.

- Please do not email protected health information (PHI). After contacting the records custodian, patients will be provided with secure instructions for identity verification and document submission.
About Northern Colorado Medical, LLC
- Northern Colorado Medical, LLC (NCM) is an independent medical practice formed after CMH ceased operations.
- NCM serves solely as the designated custodian for archived CMH patient records in accordance with applicable legal and regulatory requirements. CMH records remain separate from NCM clinical systems and are maintained in archived, read-only form.
